The Ayahuasca Ceremony, Explained: What Actually Happens, Hour by Hour

Most of what you’ll read about ayahuasca ceremonies is written to mystify you. This page does the opposite. If you’re considering sitting with this medicine, you deserve to know exactly what it is, why the preparation is non-negotiable, what the night actually looks like in a medically supervised setting, and what happens when the sun comes up — described plainly, cited where science exists, and honest about where it doesn’t.

The Brew: Two Plants That Shouldn’t Work — and Do

Ayahuasca is not one plant. It is a partnership of two, and the pharmacology is a genuine marvel. The vine — Banisteriopsis caapi — contains harmala alkaloids that inhibit the stomach’s monoamine oxidase enzymes. The leaf — Psychotria viridis — contains DMT, the God Molecule, which on its own would be neutralized by those very enzymes before it ever reached the brain. Brewed together, the vine switches off the gatekeeper and the leaf’s medicine comes through: DMT becomes orally active, unfolding over hours instead of minutes.¹ How Amazonian peoples identified this two-plant synergy among tens of thousands of species, centuries before the word “pharmacology” existed, remains one of ethnobotany’s most humbling questions. Our lineage for this work traces to the Shipibo people of the Peruvian Amazon, and we hold their tradition with the respect it commands — which begins with never treating the brew casually.

Why Preparation Is Not Optional — the Pharmacology Says So

That same MAOI mechanism that makes ayahuasca work is exactly why it demands medical seriousness. Monoamine oxidase inhibition interacts with serotonergic medications — SSRIs and SNRIs above all — with risks documented in the clinical literature since the 1990s, including the danger of excess serotonergic load.² This is why “just stop your meds for a few days before you fly” — the standard advice of the weekend-retreat world — is somewhere between useless and dangerous: these medications take weeks to properly clear, under supervision, with support.

At Holistic Sanctuary, that preparation is the program. In the weeks before any ceremony: physician-supervised in-house tapers, the daily Brain Repair IV Drip Protocol rebuilding the neurochemistry the medications suppressed, a clean superfood diet in the tradition of the ceremonial dieta, daily bodywork, breathwork, and cardio. Full cardiac screening and bloodwork before anything is scheduled. By the time your ceremony night arrives, your body has been made genuinely ready — which the tradition and the pharmacology agree on completely. The Night Itself — One Guest, One Ceremony

Here is what almost no one else will tell you, because almost no one else does it this way: your ceremony is private. Not a circle of thirty strangers. One guest — you — with your facilitator, and a physician and nurse present throughout the night. Every ceremony this facility holds is one-to-one, prepared around your own history and intentions, because the deepest night of your life should not be scheduled around anyone else’s.

The arc of the night, plainly: the ceremony begins after dark, in a dedicated space prepared for it. The brew is taken once, sometimes twice, in measured doses appropriate to your screening. Effects begin within the hour and unfold over roughly four to six. There is often purging — the tradition regards it as release, and physiologically it is common and expected; it is also precisely why trained staff and medical presence matter. There are periods of visions, of memory, of emotion — grief, joy, sometimes confrontation with the very things you came to face. Your facilitator holds the space; the medical team monitors quietly; nothing is forced, and you are never alone. Toward dawn, the medicine recedes and rest comes.

We will not script what you will experience, because no honest provider can — the medicine meets each person where they are.

The Morning After — Where the Real Work Begins

The tradition and modern psychology agree: the ceremony opens the door; integration walks through it. Here, integration is not a farewell circle and a shuttle to the airport. You wake in your own private oceanfront suite. That morning — and the days that follow — bring one-to-one integration sessions, gentle bodywork, time by the ocean, nourishing food, and the space to let what surfaced settle into meaning. Because you’re still in residence, still held, the insights of the night become decisions, and the decisions become the beginning of a different life — which is the entire point.

Ayahuasca usually closes the medicine phase of the Pouyan Method for exactly this reason: it is the deepest work, and everything before it — the weeks of preparation, sometimes ibogaine’s reset earlier in the phase — exists so this night can give everything it has.

What We Will Not Promise You

No cures — the word is banned on this website. No promised visions, no guaranteed epiphanies: the medicine owes no one a particular night. Nothing here is FDA-evaluated; ayahuasca is served legally, under licensed medical supervision, at our facilities in Mexico — never in the United States, and never to anyone our screening says it shouldn’t be. Our ayahuasca locations guide covers the legal geography in full. Certain medications, cardiac conditions, and psychiatric histories are absolute contraindications, and hearing “no” from us is a form of care. Adults only.

What we promise is the container — the preparation, the privacy, the lineage held with respect, and a doctor in the room. The medicine does what the medicine does. Our job, refined over fifteen years, is making sure that when it does, you are safe, ready, and not alone.
