5-MeO-DMT Ceremony Retreat
Reviewed by the Holistic Sanctuary Medical Review Board · Written by Johnny Tabaie, Founder & Director — 15 years of practice, 5,000+ supervised plant-medicine ceremonies

We Honor the Medicine. But First, We Honor the Temple.
Most people who find this page want to arrive and drink the medicine immediately. That is exactly what we will not do — and it is the single most important thing to understand about a 5-MeO-DMT ceremony at Holistic Sanctuary.
Most clients who come to us are on pharmaceutical medications, or they are suffering with severe physical, mental, and emotional health challenges. Handing that body one of the most intense psychoactive substances known to science on day two is not a ceremony — it is a gamble. So our program runs four to twelve weeks, and that is non-negotiable. The medical team — cardiologists, physicians, ER doctors, and ER-trained nurses — decides the timeframe for each guest, not the calendar and not the guest’s impatience. One guest at a time. One facilitator with more than 5,000 supervised ceremonies over 15 years. No groups, no crowds, no shortcuts. We honor the medicine — but first, we honor the temple.
First, We Rebuild the Temple
Before any medicine is scheduled, we bring the body back to baseline. We do not do this over a seven-day period. We take our time and we do it right: rebalancing the neurochemistry, clearing accumulated toxins, and restoring the physical foundation a profound ceremony demands.
This matters most for the guests other centers rush. A client over 45, carrying more than one mental-health condition and more than one medication — blood pressure medication, cholesterol medication — needs to be brought back to health before plant medicine is even a conversation. So that is what we do: up to nine hours of therapy a day, the juice cleanse and superfood diet, daily cardio, and our blood-pressure elixirs and cholesterol-supporting teas and nutrition protocols, all under the medical team’s supervision.
Guests taking MAOIs, SSRIs, lithium, or other psychiatric medications are not automatically turned away — this is one of the most important differences of our model. In the weeks leading up to ceremony, our medical team gradually and safely supports guests off medications and anything else that would contradict a safe journey — always supervised, never abruptly. This is exactly why we do not offer one- or two-day ceremonies: safe preparation cannot be rushed, and we will not rush it.
What Nine Hours of Daily Therapy Actually Looks Like
Each guest’s schedule is individualized by the medical team, drawn from a daily rhythm like this: an hour of therapeutic massage, an hour of Reiki, yoga and an hour of meditation, an hour in the hyperbaric chamber, and three to five hours of IV therapy — our Brain Repair Protocol, NAD+, amino acids, chelation, ozone, glutathione, vitamin C, and antioxidants — followed by an hour of infrared sauna with red light therapy, an hour in The Light System, an hour of cardio, and a 30-to-45-minute detox bath at night. Every day. That is what “we take our time” means in practice.
By the time ceremony is scheduled, the guest is not the same person who walked in — and that is the point. The ceremony receives a body that is ready for it.

Then, the Medicines — In Sequence, Never All at Once
Our treatment is never 5-MeO-DMT by itself. The Pouyan Method is a multi-faceted, multi-approach holistic protocol that weaves the sacred plant medicines together within one program — each doing what it does best, sequenced by the facilitator and medical team for the individual guest. We are one of the few healing centers in the world working with all of these medicines under one roof, inside one licensed, medically supervised program — not a one-dimensional, single-substance retreat.
When the medical team confirms readiness, ceremonial work begins with ibogaine. A few days later, when the body has settled, comes the DMT ceremony — the short, total experience this page describes. A few days after that, three nights of ayahuasca complete the ceremonial arc. Each medicine does different work: ibogaine resets, DMT dissolves, ayahuasca teaches. The spacing is deliberate — the days between ceremonies are protected for rest, bodywork, and one-to-one processing, so each experience is fully received before the next begins.
And for guests whose screening shows they are not healthy enough for ibogaine, the medical team sequences an alternative arc — psilocybin, DMT, and ayahuasca. This is not a menu you order from; it is a medical decision made for you. All a client has to do is complete the confidential enrollment application, and we will tell you honestly what you qualify for.
None of this is a medical claim. It is a biohacking and holistic approach to preparation and pre-integration — set and setting taken seriously enough to include the body itself. The temple must be healthy enough to receive the powers of ibogaine, ayahuasca, and DMT; fifteen years, five thousand ceremonies, and an unblemished safety record have taught us there is no shortcut to that. It is also what positions this work for the future — as research and regulation advance in the United States, the standard we hold ourselves to is the one we believe this medicine will demand everywhere: safety first, efficacy second.

The Therapies That Carry the Program
The ceremonies are the summit. These are the ropes that get you there and back — individualized for every guest; no two treatment plans are identical.
Brain Repair Protocol IV
Our proprietary nature-derived IV formula of natural alkaloids, peptides, and amino compounds, developed over 15 years to support the brain and body through intensive holistic programs.
NAD+, Amino Acid, Ozone, Glutathione & Vitamin IVs
Three to five hours of daily IV support, built around each guest’s labs and needs.
Hyperbaric Oxygen Therapy
Daily pressurized-oxygen sessions supporting cellular oxygenation and recovery between therapeutic work.
Chelation & Detoxification
Supervised protocols — chelation support, infrared sauna with red light therapy, juice cleansing, and nightly detox baths — designed to help the body release what it has accumulated.
Massage, Reiki, Yoga & Meditation
Daily bodywork and practice regulate the nervous system and build the one skill that matters most in ceremony: staying present with intensity.
Superfood Nutrition Program
Organic, chef-prepared cuisine and juicing designed around the program protocol, in an oceanfront setting built for rest.
The Light System
An advanced, non-invasive light technology we are proud to offer as part of the daily therapy stack — designed to support cellular function, relaxation, and nervous-system balance between sessions.

Post-Integration: Nobody Goes Straight Back Into the World
Post-integration is the phase most retreats skip — and it is where the healing either takes root or evaporates. After the ceremonial arc, the guest remains in the same healing environment where the work was done, allowing the body to acclimate to its new equilibrium and the medicine to keep working. Days keep their structure: integration sessions one-to-one, continued therapies, movement, nutrition — and the deliberate practice of new habits and new traits, so they belong to the guest before the guest goes home. Before departure, everyone leaves with an individualized integration plan and scheduled follow-up contact with our team. See our Luxury Psychedelic Retreat program for the complete picture of how the ceremonies fit into the full healing system.

What 5-MeO-DMT Actually Is
5-MeO-DMT (5-methoxy-N,N-dimethyltryptamine) is a naturally occurring tryptamine found in the secretions of the Sonoran Desert toad and in several plant species, and it can also be produced synthetically. Pharmacologically it acts primarily on the serotonin system, with strong affinity for 5-HT1A and 5-HT2A receptors — the same broad system targeted by conventional antidepressants, but engaged in a profoundly different way. Unlike psilocybin, which unfolds over four to six hours, a 5-MeO-DMT experience is short and total: onset within seconds to minutes when vaporized, a peak of twenty to forty minutes, and a return to baseline within about an hour.
Researchers at Johns Hopkins have published a narrative synthesis of the scientific literature on 5-MeO-DMT, and a Johns Hopkins survey study on PubMed found that a single 5-MeO-DMT experience occasioned mystical-type experiences of comparable intensity to high-dose psilocybin sessions — in a fraction of the time. Researchers have since mapped the minute-by-minute phenomenology of intranasal 5-MeO-DMT in Scientific Reports (Nature), and formal pharmacokinetics and safety trials registered with ClinicalTrials.gov are characterizing how the molecule behaves under controlled conditions. The experience is frequently described in the literature as ego dissolution — a temporary, complete release of the ordinary sense of self, with little imagery and little story to retell. It teaches through totality. That is why it belongs inside a prepared body, a protected setting, and an experienced facilitator’s care — and nowhere else.
The Science Is Catching Up
For most of the 15 years we have worked with this medicine, 5-MeO-DMT sat far outside mainstream medicine. That has changed. In 2025, a synthetic intranasal formulation of 5-MeO-DMT (mebufotenin, BPL-003) produced rapid and durable improvements in treatment-resistant depression in a Phase 2b clinical study. In July 2026, Eli Lilly — the largest pharmaceutical company in the world — agreed to acquire its developer, AtaiBeckley, in a deal worth up to $3.8 billion — the largest transaction in the history of psychedelic medicine. Months earlier, an April 2026 White House executive order directed federal agencies to accelerate psychedelic research and access.
We cite this research for one reason: it describes where medicine is heading, not what we promise. Holistic Sanctuary makes no medical claims about 5-MeO-DMT. What we offer is what the research setting cannot — 15 years of accumulated ceremonial and clinical experience, a complete preparation and integration system, and one guest at a time.
5-MeO-DMT and Ayahuasca: Different Teachers, One Program
Guests often ask how this ceremony differs from an ayahuasca ceremony. The honest answer: in almost every way except purpose. Ayahuasca is a long, narrative, visionary experience unfolding over hours, across multiple nights; it teaches through content — memories, images, encounters. 5-MeO-DMT is short and contentless; it teaches through totality. In the Pouyan Method they are not competitors but stages of one arc, sequenced with ibogaine so that each medicine prepares the ground for the next. Which medicines, in which order, at which spacing — that is decided for each guest by the facilitator and the medical team, never by a brochure.
Who This Is Not For
We decline guests when the fit is wrong, and we tell them plainly. This program is not for:
Guests seeking a one- or two-day ceremony. Our program is four to twelve weeks. This is non-negotiable. Preparation and integration are where safety and meaning are built, and we will not compress them for anyone — at any price.
People with childhood-onset schizophrenia or psychosis. Screening will identify this history, and the honest answer is that this medicine is not a good fit.
Clients experiencing drug- or medication-induced psychosis or schizophrenia. Stabilization must come first, and a psychedelic ceremony is not the setting for it.
If screening finds the fit is wrong, we will help you understand the alternatives — including whether a non-ceremonial luxury treatment program is the safer path. Saying no is part of the safety record.
Admissions
Admission begins with a private, confidential conversation — not a sales call. You will speak with our team about your history, your medications, and what you are trying to heal. Medical screening follows, then the medical team recommends your program length and structure. Our facilities host a strictly limited number of guests at a time, and ceremonies are scheduled around each guest’s program, not a calendar of group events.

5-MeO-DMT Retreat FAQs
Is a 5-MeO-DMT ceremony legal at your facility?
Yes. Our ceremonies are conducted at a licensed facility in Mexico, where this work may be legally offered in a medically supervised environment. This is one of the central reasons the facility is located where it is.
How long is the program?
Four to twelve weeks, determined by our medical team during screening. We do not offer one- or two-day ceremonies — preparation and integration are the foundation of both safety and results, and they cannot be compressed.
Why can’t I just do the ceremony in a few days?
Because most guests arrive on medications or in depleted health, and the body must be brought back to baseline first. The pre-ceremony weeks are where we safely taper medications, rebalance neurochemistry, and clear what would contradict a safe journey. We honor the medicine — but first, we honor the temple.
How long does the 5-MeO-DMT experience itself last?
The acute experience typically peaks within minutes and resolves within about an hour — far shorter than psilocybin or ayahuasca. The full ceremony day is protected for preparation, the experience, and supported rest.
Can I attend if I take antidepressants, MAOIs, or lithium?
In many cases, yes — this is precisely why our program is four to twelve weeks. Our medical team prepares each guest correctly, gradually and safely supporting them off medications that would contradict a safe journey. The medical team decides the timeframe, individually.
Is the ceremony only 5-MeO-DMT?
No. The Pouyan Method sequences ibogaine, DMT, and three nights of ayahuasca within one program, spaced deliberately, with the daily therapy stack carrying the guest between ceremonies. This is a multi-faceted protocol, not a single-substance retreat.
How is this different from a group psychedelic retreat?
Everything here is one-to-one: one guest, one facilitator, one medical team, one protected ceremony space. We do not run group ceremonies at any price point.
