Role and Responsibilities
Johnny Tabaie’s role at Holistic Sanctuary is operational and protocol-level. He is responsible for the design and sequencing of the programme, for admissions decisions, for the standard the facility holds itself to, and for the day-to-day running of the centre. He is the person who determines whether a prospective guest is a fit for the model and, frequently, the person who declines.
He does not diagnose. He does not prescribe. He does not administer medical procedures or make clinical decisions. Those responsibilities sit with the Medical Director, Dr. Jose A. Jimenez, MD — a visiting oversight and reviewing physician who medically clears each client and approves the treatment they are placed on — and with the in-house clinical staff who deliver the care: nursing cover 24 hours a day across a morning shift, an evening shift and two to three night shifts, with at least four nurses on duty across a 24-hour period, plus three rotating ER doctors who review bloodwork, medically supervise the ibogaine treatment, and sign off on protocols involving stem cells, autoimmune conditions, diabetes, cholesterol and tapering off prescribed medication. An ER doctor is also present for any session where a client is frail or medically compromised.
The division is deliberate and it is not negotiable internally. A protocol architect who also decides who gets what drug is a conflict of interest with a nice title on it.
The Protocol He Authored
The Pouyan Method is the sequenced protocol he developed and has revised continuously since the facility opened. Its defining characteristics are a four-to-twelve-week duration rather than a fixed short-stay clock, one-to-one delivery with no group component, a medication-free destination reached through in-house, medically supervised tapers, and plant medicine placed at the end of a stabilisation sequence rather than at the start.
Ibogaine, where it is used at all, is always last. That ordering exists because ibogaine carries the most serious cardiac considerations of any compound the facility works with, and because the published outcome literature on it comes from screened, supervised settings.2

Credentials, and the Limits of Them
Everything Johnny Tabaie knows about addiction and psychiatric medication dependency he learned first as a patient and then as an operator. He was prescribed psychiatric medication at thirteen and went through seventeen treatment programmes before building his own. He lost his brother to suicide inside a licensed treatment facility. What that history is not is a medical qualification, and this profile exists partly to state that in a place search engines and prospective guests can both find it. He has no medical degree, no nursing licence, and no clinical certification. Any account of Holistic Sanctuary that describes him as a doctor, a clinician, or a medical operations leader is inaccurate, and we would ask that it be corrected.
Position on the Research
Johnny Tabaie’s public position on the science is consistent and deliberately unexciting. The 2007 hypothesis behind the protocol has moved from fringe to institutional interest — Eli Lilly agreed in July 2026 to acquire AtaiBeckley for up to $3.8 billion, centred on a synthetic 5-MeO-DMT nasal spray in late-stage trials.1 He treats that as evidence that the direction was right, not as evidence that his protocol works.
He does not claim a cure, does not quote success percentages, and does not describe the facility as the only one of its kind without qualifying what he means. How the Role Works Day to Day
His day is not the founder’s day the title tends to imply. It starts with the guest in programme rather than with the business — the morning check, the previous night’s sleep and vitals as reported by nursing, and whatever came up in the hours he was not there.
Admissions occupy the middle of it. He runs the screening conversation himself: medication profile, cardiac history, expectations, and the harder question of whether the person actually wants to be here. He is also the one who says no when the answer is that the model does not fit.
The remainder goes to the decisions the sequence throws up. Whether a taper moves this week or holds. Whether a guest is ready for the ceremony phase or needs another fortnight of preparation. Whether a component needs revising for the person in front of him. Every clinical judgement inside those decisions belongs to the Medical Director and the licensed staff; the sequencing and the standard belong to him.

Anyone whose cardiac screening, bloodwork or medication profile makes the sequence unsafe is declined, and no amount of willingness on the guest’s part changes that — the screening is not a formality that can be waived. Anyone looking for a one-week stay is declined, because the shortest programme here is four weeks and compressing it would produce the exact outcome the facility was built to avoid. Anyone who wants to arrive and go straight into ceremony is declined for the same reason: the preparation phase is not a waiting room.
And anyone seeking a guarantee is told, plainly, that there isn’t one. Individual results vary, some people do not respond, and a founder who promises otherwise is a founder who will eventually have to explain himself to a family.
