Why I Built a Program for This
Benzodiazepine dependence is the case that made me stop believing in the short program. A person can be admitted, medicated, discharged in a month and told they are “detoxed,” and be worse in week six than they were on the day they arrived, because the drug was removed faster than the brain could adapt. I have watched it happen to people who had done everything right and were told the problem was them. That is the version of benzodiazepine detox in California most people are offered, and it is the version I refuse to run.

What California Already Gets Right, and Where It Stops
California takes these drugs seriously, and it deserves credit for that. Every prescriber and pharmacist in the state must be registered with CURES, the Controlled Substance Utilization Review and Evaluation System, which records every Schedule II through V prescription dispensed in California. Under Health and Safety Code section 11165.4, a prescriber must consult a patient’s CURES history before prescribing a Schedule II, III or IV controlled substance for the first time, and at least every six months while that prescription continues.
Since January 1, 2022, under Business and Professions Code section 688, prescriptions issued to a California pharmacy must be transmitted electronically, and controlled-substance e-prescriptions must meet federal DEA requirements. And since January 1, 2021, Senate Bill 855 has required state-regulated commercial health plans to cover medically necessary treatment for mental health conditions and substance use disorders without limiting those benefits.
Here is where it stops. Every one of those rules governs how a benzodiazepine is written, tracked and paid for, and none of them is a plan for benzodiazepine detox in California. Not one of them governs how a person is helped off it. A doctor who consults CURES twice a year and sends a prescription electronically can still cut a dose in half at a monthly visit, and the state has nothing to say about it. Benzodiazepine detox in California is regulated at the pharmacy counter and unregulated in the part that matters to you.
What the Numbers Say About California
The California Department of Public Health counted 11,002 drug-related overdose deaths in the state in 2022, an age-adjusted rate of 27.1 per 100,000 residents. Of those, 676 involved a benzodiazepine in combination with another drug, a rate of 1.7 per 100,000. In 2023 the number rose again, to 11,359 by the California Health Policy Strategies analysis of state data, more than double the 5,401 deaths of 2018. Federal figures agree on the direction: the CDC’s National Center for Health Statistics put California at 27.9 overdose deaths per 100,000 in 2023 and named it one of only six states where the rate increased that year.
Those are overdose numbers, and a person going through a supervised benzodiazepine detox in California is not usually in them. I include them for a different reason. They are why California’s rules have become stricter every year, why prescribers are more reluctant to continue a benzodiazepine, and why a person who has been on one for years increasingly finds the door closing behind them rather than a plan in front of them. Question One: Who Writes the Taper, and How Often Is It Adjusted?
This is the question that separates programs, and it is the first one I would ask anyone offering benzodiazepine detox in California. The joint clinical practice guideline published in 2025 by the American Society of Addiction Medicine with nine other medical societies says that anyone who has taken a benzodiazepine for longer than a month should not stop abruptly, and should instead taper gradually under clinical supervision, with a schedule that is individualized and adjusted as symptoms change.
Adjusted is the word. A taper written once and followed to the end is not a taper; it is a schedule. In outpatient care the person writing it sees you every few weeks, which means every change waits for the next appointment, and a bad week gets managed by you alone. Question Two: How Long Is Benzodiazepine Detox in California, and Who Decided?
For a Californian, benzodiazepine detox at Holistic Sanctuary takes twelve to sixteen weeks, the same as every benzodiazepine program we run. That is not a marketing number and it is not a package; it is the length of time a slow, individualized taper takes when it is done at the pace the nervous system tolerates rather than the pace a discharge date requires. The Ashton Manual, which remains the most widely read taper guide in the world, describes withdrawal schedules that commonly run months rather than weeks, and describes patients who needed longer.
Most benzodiazepine detox in California is offered in a twenty-eight-day frame because that is the frame insurance was built around. I have written elsewhere about why twenty-eight days fails, and the short version for benzodiazepines is arithmetic: a month is not long enough to bring a person from a therapeutic dose to zero without a rate of reduction that the guideline itself warns against. Question Four: What Is Not Done, and Why
What a program for benzodiazepine detox in California refuses to do tells you more than its brochure. There is no group therapy here. There is no talk therapy offered in place of medicine, and no psychobabble. There is no strategy of replacing a benzodiazepine with a different dependence-forming medication and calling the person treated. There is no fixed schedule that ignores how you feel. There is nothing administered to any guest before a full medical screening, and nothing administered without a physician’s order.
Two Cochrane reviews are the reason for that restraint, and I would rather you hear about them from me than from nobody. A 2018 review of thirty-eight trials of medications intended to ease benzodiazepine withdrawal found the evidence too weak to draw firm conclusions. A 2015 review found that adding cognitive behavioral therapy to a taper helped at three months, but the benefit did not persist beyond six. Benzodiazepine detox in California, or anywhere, that promises a pill or a workbook will fix this is promising something the research does not support.

Question Five: Who Do You Turn Away?
A program that admits everyone is not screening anyone, and that is as true of benzodiazepine detox in California as it is here.
